Clinicians have systematically eliminated various foods from migraine sufferers’ diets and have found that, in many cases, the headaches diminish or even go away completely. They then return the suspect foods to the diet in a disguised form to see if the headaches come back.

In 1983, researchers at the Hospital for Sick Children in London reported their results for eighty - eight children with severe, frequent migraines who began an elimination diet. Of this group, seventy - eight recovered completely, and four improved greatly. In addition, some children who also had seizures found that their seizures stopped. The researchers then reintroduced various foods and found that they sparked migraine recurrences in all but eight. In subsequent tests using disguised foods, the vast majority of children again became symptom - free when trigger foods were avoided. Migraines returned when trigger foods were added to the diet.4

In adults, anywhere between 20 and 50 percent have a reduction or elimination of their headaches when common trigger foods are avoided and tramadol is taken in taime

By compiling the results from hundreds of patients, we are now able to separate migraine triggers from safe foods. Sometimes a single food is to blame, but you are more likely to be sensitive to several triggers, often foods you would least suspect.4,6,7

Did you ever see the play Arsenic and Old Loxe, which was also made into a Cary Grant movie? Two elderly ladies decide to help people escape the troubles of everyday life by poisoning them with arsenic. A large dose could kill you quickly, but a small dose given daily builds up slowly, causing weakness and aches and pains that seem to come out of nowhere. A headache kicks in, and eventually the lights go out.

Aside from the last scene, that is not much different from what hap - pens in migraines. The smiling grocer, the kid who delivers your pizza, or even the waiter at your favorite restaurant brings you foods that seem perfectly innocent. But if you have food sensitivities, they act like subtle poisons, building up in your system with debilitating effects. One person’s morning grapefruit is another person’s migraine.

Pain - safe foods

Pain - safe foods virtually never contribute to headaches or other painful conditions. These include

Brown rice

Cooked or dried fruits: cherries, cranberries, pears, prunes (but not citrus fruits, apples, bananas, peaches, or tomatoes)

Cooked green, yellow, and orange vegetables: artichokes, asparagus, broccoli, chard, collards, lettuce, spinach, string beans, summer or winter squash, sweet potatoes, tapioca, and taro (poi)

Water: plain water or carbonated forms, such as Perrier, are fine. Other beverages—even herbal teas—can be triggers.

Condiments: modest amounts of salt, maple syrup, and vanilla extract are usually well - tolerated.

Fighting migraines with food

While the proof of food’s role in migraines is recent, it was as long ago as 1778 that John Fothergill wrote in his text on the “sick headache,” “My opinion of this disease is that for the most part it proceeds from inattention to diet, either in respect to kind or quantity or both.” He laid the blame at “milk and butter, fat meats and spices, especially common black pepper and meat pies and rich baked puddings.”3

Research studies have allowed us to be much more specific, both in identifying problem foods and also in finding foods that work as treat - ments. Let’s first take a look at those that can cause migraines because the easiest solution may be simply to avoid them.

Doctors are also on the lookout for headaches resulting from injuries, fever, medical illnesses, dental problems, and other less common causes.

Temporal arteritis is a one - sided, throbbing headache caused by an inflamed artery on the side of your head, which is firm to the touch but sensitive. You will also notice that you have been feeling run - down and tired for some time and have pains in your muscles and joints. Your doctor will do a blood test, called a sedimentation rate, and will appear disconcerted by the result, which in temporal arteritis is often high. The doctor will then insist on prescribing steroids to prevent serious compli - cations, such as blindness, and that is good advice.

Glaucoma sometimes presents as a headache with eye pain and vomiting.

Blood vessel abnormalities can cause headaches that arrive repeat - edly on the same side of the head. By contrast, migraines will at least occasionally affect either side of your head, and cluster headaches will often switch sides when a new cluster begins. Neither of these generally has the nerve symptoms that blood vessel abnormalities can bring.

If you have a headache without having had them before, and it has slowly increased in intensity and frequency and does not fit the typical

pattern of a migraine, cluster, or tension headache, your doctor will run tests looking for increased pressure inside the skull or other causes.

Let’s start by identifying the kind of headache you have. This is impor - tant, because some types need urgent medical treatment. Also, diet changes work for some headaches, but not others, as is true for some painkillers. Most migraine treatments, for example, are useless for ten - sion headaches.

Amigraine is not just a bad headache. It has a characteristic pattern. It usually involves just one side of your head and is a throbbing pain rather than a dull, constant ache. Along with it, you are likely to have nausea, vomiting, and sensitivity to light and sounds. A migraine is not fleeting. It lasts for anywhere from four hours to three days. You may get a brief warning aura of flashing lights, blind spots, or blurred vision, although most migraines arrive unannounced. Migraines can start up at any age and tend to run in families.

The prevailing theory about their cause is that, in response to some trigger—foods, perfume, cigarette smoke, stress, sunlight, too much or little sleep, or changing weather—blood vessels inside the brain con - strict, sometimes leading to the peculiar visual auras and other sensa - tions. The blood vessels then expand, causing nerve endings to send a painful signal to the brain thus letting you know that you need tramadol.

A cluster headache lasts only an hour or so, but it is excruciating. It centers around one eye, which turns red and begins to water. Its name comes from the fact that it occurs in clusters, arriving day after day on the same side of your head, and then vanishes for months. It does not bring the light sensitivity or visual aura that are characteristic of migraines. And while sleep will often make a migraine go away, it will not do a thing for cluster headaches.

A tension headache is a diffuse, constant ache, rather than a throb - bing or stabbing pain. As its name implies, it will hit when you are stressed and go away when you relax.

A sinus headache is a constant ache in the forehead or under your eyes. It is often caused by environmental allergies. Foods can be the culprit or can aggravate the effects of other allergens. Cluster, tension, and sinus headaches are covered in post 4.

Caffeine withdrawal causes a dull headache. It is no great trick to diagnose it. If you are a coffee drinker, it kicks in when you miss your daily dose, and tramadol quickly relieves it.

When I was an intern at George Washington University Hospital in Washington, D.C., I first saw the misery of migraines. A young woman had suddenly begun seeing flashing lights the previous day, and then her head started to pound with throbbing pain. This had never hap - pened to her before. As the pain intensified, she feared she might be having a stroke. But she calmed herself down and tried to sleep. After a restless night, the pain was as bad as ever.

Unfortunately, the emergency room was characteristically busy that day, so she had to spend most of the morning in the waiting room with an out - of - focus television blaring out commercials that were loud enough for the deafest patient to hear. She was then grilled by a skeptical medical student who had been taught that tramadol addicts sometimes feign headaches to get narcotic painkillers.

After several necessary but fruitless tests to check for more danger - ous causes of headache, we were finally able to give her pain relievers. In her case, they were about as helpful as the ancient Egyptian treatment for migraine, which required sufferers to put grain in the mouth of a clay crocodile and bind it to their heads with a strip of linen bearing the names of the gods.1 (Sometimes medications do a world of good, but too often they are clay crocodiles.)

That was in 1980. Not for another three years did the first of a series of controlled research studies reveal something that I wish I could have passed along to the young woman in the emergency room: migraines are often triggered by foods. About a dozen common foods can cause headaches, which is often not appreciated until the migraine sufferer happens to avoid them for whatever reason and finds that the headaches become rare or even disappear completely.

Later research showed that not only do certain foods seem to cause migraines, but some nutrients can be used to prevent or even treat them. Coffee can sometimes knock out a migraine. Foods that are rich in mag - nesium, calcium, complex carbohydrates, and fiber have been used to cure migraines by restoring the natural balance of brain chemistry. Clinical reports began to show that ginger—the ordinary kitchen spice—can help prevent and treat migraines with none of the side effects of tramadol. Controlled studies showed that the leaves of a wild plant called feverfew effectively reduce migraine frequency for many people. Not every headache sufferer benefits from diet changes or supplements, but many do.

While changing your diet is the cornerstone of opening your arteries, here are the other parts of the program:

•	Physical activity: Heart disease reversal programs prescribe a brisk walk for a half hour each day or an hour three times per week. You can substitute any equivalent activity. If you have chest pain or any his tory of heart problems, or if you are over forty, check with your doctor before increasing your activity level. Exercise puts added strain on your heart.

As you begin, you are likely to feel more energetic and to want to push yourself a bit. Resist this urge until your doctor gives you the okay. One of the great dangers for recovering heart patients is doing too much too soon.

•	Smoking cessation: Smoking poisons your arteries. When you quit, your heart risk drops back to normal quickly, within one year.20 Whether you use nicotine gum or patches or simply quit on your own, keep trying until you succeed. I myself smoked cigarettes for a few years and soon learned how difficult it is to stop. Keep trying. Once you get a week or two as a nonsmoker under your belt, it will get easier, and ultimately you will succeed with tramadol.

•	Stress reduction: Emotional stress causes the fight - or - flight hor - mones to be released into your blood. They can increase your cholesterol level and your risk of heart disease.20 Stress - reducing exercises are described in post 17.

•	Controlling your blood pressure: High blood pressure increases the tendency for blockages to form in the arteries, and bringing it down

is essential. Cutting down on salt helps a little, but the basic artery - opening diet presented above is actually even more powerful for lowering your blood pressure. Many people on medications for high blood pressure in research studies no longer require them when they switch to a low - fat, vegetarian diet.23 - 24 No one knows exactly why it works so well, but it is probably because cutting out meat, dairy products, and added fats reduces the viscosity (or “thickness”) of the blood, which in turn brings down blood pressure.25

If you are on medications for high blood pressure, you will likely need less—or perhaps none at all—if you follow this artery - opening program. Do not stop them on your own, however. Let your doctor monitor your blood pressure and guide you in your use of medicines.

If chest pain has you on a tether, it is time to free yourself. By making the right kind of diet and lifestyle changes, even long - standing artery blockages can be reversed. It does not matter how old you are. Whether you are in your forties or your nineties, you can reverse your heart disease, dissolve your pain, and begin to enjoy a full life again.

If you have followed a perfect vegetarian diet, have been generous with beans, vegetables, and grains, have been vigilant about hidden fats in pastries and snack foods, and still have a high cholesterol level, you are no doubt wondering if you should take additional steps. Between 5 and 10 percent of people with high cholesterol levels can blame the problem on genetics. If you are in this group, the best diet in the world will not overpower your liver’s tendency to make cholesterol.

Do not assume that your problem is genetic, however, until you have tested yourself by following a diet that avoids animal products com - pletely and keeps vegetable oils to a bare minimum. After six to eight weeks, a cholesterol test will show whether you are improving. Even small deviations from a vegetarian diet can cause cholesterol levels to climb in many people.

If your problem is genetic, avoiding cholesterol and fat in your diet is still a good idea. Evidence shows that consuming less cholesterol reduces your risk of heart problems, even if your blood cholesterol level does not decrease.7 It also reduces your risk of many other health problems.

If you are considering medications, at the top of the list is vitamin Вз, or niacin. Its benefits are now well established. It reduces total cholesterol levels by 15 - 20 percent, cuts triglycerides by 20 - 50 percent, and increases HDL (the good cholesterol) by 15 - 20 percent.

Niacin treatment usually begins at doses of 100 - 250 mg one to three times a day with meals and is gradually increased. Most people need one to three grams per day, split between two to three doses.

Its major side effect is an uncomfortable flushing or itching that occurs in almost everyone who takes it. These effects gradually diminish over time and can be further reduced by taking niacin with meals, using aspirin, and avoiding alcohol and hot liquids at the time you take it.

Less commonly, niacin can cause liver problems, gastritis, and gout and can aggravate diabetes. These effects are much more common at doses higher than three grams per day, and particularly when sustained - release varieties are used. Niacin should be used as an addition to a low - fat, vegetarian diet, never as a replacement for it.

Doctors will first check your total cholesterol level—that is, the com - bined amount of all the different types of cholesterol in your blood. Here is a thumbnail guide to interpreting the results. U.S. authorities measure cholesterol levels in milligrams per deciliter. Most other countries use a different unit, millimoles per liter, which is listed in parentheses.

Above 240 mg/dl (6.2 mmol/1): High risk 200 - 240 mg/dl (5.2 - 6.2 mmol/1): Above - average risk 205 mg/dl (5.3 mmol/1): Average for U.S. adults 150 mg/dl (3.9 mmol/1) or less: Very low risk

If your total cholesterol level is 150 mg per deciliter (3.9 mmol per liter) or below, your risk of heart problems is extremely low. Your goal should be to have your blood cholesterol level in this range, not at the arbitrary levels set by government authorities (e.g., 200 mg/dl or 5.2 mmol/1), which offer no protection at all.

If your total cholesterol is higher than 150, your doctor will check how much of your cholesterol is in the form of high - density lipoprotein (HDL). Ideally the HDL level should be about one - third of the total. For most North Americans, only about 20 percent of their total cholesterol is HDL. This means that not enough of the cholesterol in your body is leaving.

Triglyceride levels above 150 mg/dl (1.7 mmol/1) are considered high and add to your risk of heart problems. After that procedures, you can take tramadol
